Athletic Eligibility	Attendance Reminder
Students who want to try out for Vickery Creek Middle School Athletics must satisfy the following requirements before they can tryout: 1. Students must pass 5 of 7 or 6 of 8 classes to be eligible to tryout for any sport per last semester. 2. Students must have an updated insurance and physical form completed to be eligible to tryout for any sport. These forms are only valid for one year from the date of the physical.	In order to participate in an extra-curricular event, including team try-outs, a student must be present at least half of the school day according to the school attendance office.
VCMS Sports News	Athletic and Activities Passes
We will be using GoFan for our tickets this season. Please use the following link to purchase tickets: https://gofan.co/app/school/GA74250.	The FCS activities and athletics passes are now available for both seniors/retirees and veterans. These passes include half-price admission for one to FCS home athletic events, musicals, plays, band concerts and other special school-sponsored activities. Half-price admission does not extend to play-off games, graduation or PTO/A sponsored events. Passes are issued at the Central Office. Physicals

All students trying out for any sports at the middle or high school levels must have a current physical completed by a doctor and uploaded to DragonFly. You can get a Forsyth County physical form from the front office staff or you can download it by clicking on the link located below.

Download the DragonFly MAX app from the App Store or Google Play. You can also create an account from your personal computer by going to https://www.dragonflymax.com/.
Tap “Get Started” and “Sign Up for Free” then follow the prompts to create your account with your email address. Please do not create an account with your child’s name or contact information. There will be an opportunity to add your child at another point.
Verify your account with the verification ID sent to your email address.
Tap “Connect to your school” to select “Parent” as your role and search for your child’s school.
After selecting your child’s school, tap “Join” to request access. A school administrator will approve your request.
Tap “Set up your children” and follow the prompts to add your kid(s) and fill out the eight mandatory forms.
The following mandatory forms are completed electronically in DragonFly:

- Medical and Demographic Information
- Blanket Permission to Participate
- Cardiac Arrest Awareness Form
- Parent Concussion Awareness Form
- GHSA Heat and Humidity Policy
- Health History Form

The following forms need to be uploaded in DragonFly:

- Insurance Card (front and back)
- Physical Examination and Medical Eligibility Forms (forms can be found at the top of this page and need to be uploaded to Dragonfly together)

A school administrator will approve your forms. If a form is “Rejected”, there will be a comment added to let you know what needs to be corrected.

Medical Insurance
